Individual Registration

Students must meet the eligibility requirements listed below for consideration in either the Individual Online or Nationals competition.
 
Eligibility requirements

  • Students must not be part of the state championship team attending Nationals
  • Students must not be part of a large, medium and/or small school team who qualifies to compete in the USAD Online large, medium and/or small school competition.
  • Students interested in the opportunity to be invited to Nationals in Wisconsin must have had their state competition prior to March 26 so they can have their coach submit their state objective scores on or before March 22 using the Excel spreadsheet in Step 2. California scores are due March 31.

Individual Registration Process

Please register on the USAD® Individual Competition website if you have not registered on the site. Register for round 4 at http://usadic.org/Registration-Rnd4.aspx. (Registration will open on the website for USAD® Individual Final - Nationals on March 1)

Students: Please note the 2017 Online Testing Schedule, Online Technical Information, Competition Guidelines, and Code of Conduct.

Step 1: Students complete the 2017 Individual Registration Form.
Due:  No later than 
March 19, 2017 unless your competition is March 16 or after, the form is due March 22. California forms are due March 31. 

Step 2: Coaches please submit your decathlete’s information using this Excel spreadsheet link (you may list all your students on one spreadsheet if you have more than one participating).  These scores can be found on the official state competition reports.
Due: No later than March 19, 2017 unless your competition is March 16 or after, the form is due March 22. California forms are due March 31. 

Step 3: Students who register after their state competition or those who have been invited and have accepted to participate as an individual at Nationals in Madison must mail your transcript, GPA calculations, and the following forms to USAD Individuals Competition, PO Box 4351, Mankato, MN 56002. Forms with an asterisk are only required for individuals who participate in Madison. (Please mail the forms in the order requested below.)
Students please note – we are requiring a Calculator Use Form this year. Please refer to the USAD Calculator Policy for Curriculum year 2016-2017 if you have questions.

If you have been invited to attend USAD Nationals in Madison, WI please refer to the “Welcome Letter.” It will answer your questions about the competition, lodging, transportation, meals, protocol, etc. If you have accepted the invitation to attend Nationals in Madison, please complete the following steps:

Step 4: Please be sure to remit your $250 participation fee. Payment information will be included in your invitation email.

Step 5: Individuals make sure your online testing proctor completes the USAD Online Proctor Security Affidavit Form by March 31, 2017. (Please substitute the decathlete’s name in place of the school name on the affidavit.)

Step 6: Hotel Accommodations, Transportation & Competition Site - Individuals are responsible for making their own hotel arrangements. 

This year's host hotels are the Hilton Madison Monona Terrace (Hilton), 9 East Wilson St., Madison, WI 53703 and The Madison Concourse Hotel and Governor’s Club (Concourse), 1 W. Dayton St., Madison, WI 53703, USA. Rooms are limited at each property and reservations will be processed on a first-come, first-serve basis. 

#1: Reserve your room at Madison Concourse Hotel (based on availibility):
Due: 12:00 p.m. CT April 7, 2017 

Concourse: Complete the Concourse Guest Reservation Form and submit directly to the Hotel. (Directions for submission are on the form.)
 
#2: The Concourse has complimentary airport shuttle service available (gratuities suggested) to the hotel – please contact them when you have retrieved your bags and are ready for pickup. Rental cars and vans are also an option. If renting a car/van at the airport and driving to the hotelt the Concourse offers parking at $10/day for overnight guests or $1.50/hour to a maximum of $17 per day.  Public parking lots are available at reduced rates, please view map

2017 COMPETITION SITE: All academic testing will be held at the Monona Terrace Community & Convention Center (Convention Center), One John Nolen Drive, Madison, WI 53703.

Step 7: Complete the Extras Form – T-shirt, additional breakfast and banquet tickets order form.
